Dobutamine Stress Echo is a specialized form of echocardiography that aims to look at how the heart is reacting to graded stress, which gives a clue to the underlying disease process.

Dobutamine Stress Echo is generally done in patients who are unable to walk or exercise. However, it can also be done in patients to see reversible Myocardial ischemia and to check for culprit artery or which artery to do first in a case of staged procedure and sometimes, even to check for viability and the need for procedure.

Every center has their own protocols of doing a particular test.

Agra Heart Center, under the guidance of Dr. C. R. Rawat, President of UP chapter of Indian Society of Cardiology and President of RSSHI (Research Society for the Study of Hypertension in India) along with Dr. Rajat Rawat (Consultant and Interventional Cardiologist, MD, DM Cardiology) have evolved the protocol to aim for excellent patient comfort and to allieve any anxiety due to the procedure.

If your doctor has prescribed you a dobutamine Stress echo:

Consult with your doctor whether you need to be fasting or not.

At Agra Heart Center, we prefer the patient to be atleast 4 hours of fasting before the procedure. If you’re taking breakfast, take a light meal which is non-fatty.

A consent form signed is a must before any procedure and part of the protocol.

An IV cannula is placed and baseline ECG and 2d echo is obtained.

Patient is connected to the monitor and continuous ECG recording.

The IV dobutamine is started at graded level, starting from 5 mcg/kg/min and increased slowly at 5-10 mcg/kg/min increments to a maximum of 40 mcg/kg/min.

At Agra Heart Center, all procedures are done under the watchful eye of the cardiologist, Dr. Rajat Rawat or Dr. C. R. Rawat.

All symptoms and signs are noted.

Continuous BP and Heart rate is monitored during the procedure and eve after it, till heart rate returns to baseline.

Continuous echo recording is done to check for wall motion abnormality, increase is LV dimensions, increase or decrease in Ejection fraction or mitral regurgitation and E/e’ or LV diastolic function.

Your doctor may need to do a few more parameters or maybe a little less depending on the purpose of the test.

Dobutamine Stress Echo is also an excellent test for preoperative cardiac risk assessment in case where patient is not able to walk or exercise, before going for a non-cardiac surgery.

Agra heart Center was the 1st hospital to introduce dobutamine Stress Echo in Agra, and has been conducting the test successfully for few decades now and thousands of patients have undergone dobutamine Stress Echo successfully at Agra Heart Center.
