CRT-D / CRT-P is cardiac resychronization therapy with defibrillator or overdrive pacing.
It is a special form of device that is used in patients with Heart Failure with low Ejection fraction (low pumping power of the heart).
It has certain pre-requisite criterias for implantation. Maximal benefit is seen with patients with low Ejection fraction poorly controlled symptoms on medical management that have wide LBBB (left bundle branch block). However, there are many other patient subtypes that can derive benefit from the device.
CRT-D / CRT-P causes the heart chambers to maintain synchrony and gives them impetus for a good strong contraction that improves the Ejection fraction or pumping power of the heart which alleviates the symptoms of heart failure.
Patients with heart failure with low EF are at risk of sudden cardiac death – the defibrillator component in the CRT -D device gives appropriate shock so as to prevent sudden cardiac death when needed.
CRT-P device instead of giving shock, gives a burst or train of rapid pacing (electrical stimulus) to solve the problem of Ventricular tachycardia that may cause sudden cardiac death.
CRT-D /CRT-P device is installed via a small cut in the left/ right chest, under local anesthesia. With a small access taken from the leg/ arm so as to engage the coronary sinus to implant the leads that maintain synchrony.
CRT device batteries usually last for about a decade or maybe higher/lower depending on the usage. After which the batteries can get replaced.
Patients are encouraged to continue taking their heart failure medications with other precautions. Many patients have dramatic improvements in their heart failure symptoms after device implantation.
