It is always a great feeling to be able to deliver the best results to the patients, especially by using state of the art medicines and devices.
One such case, was that of a patient, who was referred to our centre, Agra Heart Center, Agra, from a Tier III city, with huge ST elevation in the anterior-precordial leads and severe excruciating chest pain.
Dr. Rajat Rawat (MD, DM Cardiology) immediately attended to the patient, and started supportive medications for the heart attack. As the patient, was out of window period, He was offered upfront Angiography and Angioplasty. This was going to be a high risk case. However, to save the patient’s life, angioplasty was attempted.
The Angiography showed a 100% occluded (Totally blocked) Left anterior descending artery with high thrombus burden. Dr. Rajat Rawat with his vast experience of doing more than 750 angioplasties and thousands of angiographies, and managing heart attack patients, realised that patient might require thrombo-suction device during angioplasty so as to remove the blood clot, however, as patient was not from a very good financial background, doctor did not want to escalate the cost also. Hence, after discussion with the family, and understanding their financial situation, decision was taken to use the novel drug- cangrelor, as the patient was not previously anti-plateleted at the outside center.
Cangrelor was started, adhering to it’s strict dosage regimen and method of preparation and was given throughout the angioplasty time. Thrombus softening was noted after about 10-12 minutes, and after dottering of the unexpanded balloon, thrombus started to clear. Dottering before softening of the thrombus wasn’t advisable, as thrombus migration to LCx would have proved catastrophic. When thrombus was cleared, Patient underwent PTCA and stenting to LAD with excellent results.
Thrombo-suction device would have setback the patient by about 30k, howeverm cangrelor costed about 7500 only and also, gave excellent results with no risk of dissection that might have been caused by the thrombosuction device. Hencem decreasing the risk to the patient,
Dr. Rajat Rawat, Senior Interventional Cardiologist, is the first cardiologist in Agra to use cangrelor in the city of Agra.
